Overview

During onboarding, provide the forms the organization needs.

After our Forms Team builds them and notifies you, review each form to ensure it’s set up for its intended use.

• Confirm the following:
  • Who completes the form (student, site supervisor, faculty)
  • The form’s purpose is met
  • Conditional fields appear correctly (e.g., follow-up text for Yes/No choices)
  • Signature fields are present
  • Signing order starts with the primary completer and proceeds to reviewers.
• After review, request any needed edits.
  • You can request edits by submitting a service request or you can edit forms yourself with the Program Setup role.
  • If you are submitting a form edit request, please be sure to provide the form name, number, and specified changes within your request.
  • Note: Edits take time and if you know you will need changes made before a certain date, submit your edit as soon as possible.
