Overview

Electronic forms in Experiential Learning Cloud can be completed by students, site supervisors, or faculty members. Experiential Learning Cloud provides a selection of forms and allows users to create custom forms.

Purpose of Document

This article describes the process of creating a form assignment in a class template. Click here to learn more about class templates. 

Creating a Form Assignment 

1. Go to the Assignments tab in your class template
2. Click (+) to create a new assignment
3. The assignment builder will populate

Required Settings

• Name: Enter the name of the assignment.
• Assignment Type: Select Form as the type of assignment.
• The form to be used is: Click the dropdown menu and select the form you need. When you select a form you can click on the Preview Form button to preview the form you are attaching to the assignment. 
  • To view the contents of all forms available to your program, you can also go to the Forms Workspace in your Customization Environment. 
• IMPORTANT: Set who the assignment is About.  
  • The About setting determines where the assignment will be filed in your Experiential Learning Cloud database.
  • The vast majority of documents in Experiential Learning Cloud should be about the Student (assignee). One notable exception would be in cases where the student is evaluating their site supervisor.
• IMPORTANT: Set who the assignment is Completed By.  
  • The Completed by setting determines who will be the first person to fill out the form.
  • Selecting "Student (assignee)" would allow the student to fill out the form first. 
    • If an assignment is completed by students, the "allow student resubmission" checkbox will appear. If student resubmission is allowed, students can make changes to an assignment after initially submitting it. If the original assignment was signed or reviewed by anyone, they will need to re-sign or review the updated version.  
  • Selecting "supervisor" would allow the site supervisor to fill out the form first (once the student delegates it to them). 
  • Selecting "faculty" would allow you as the course instructor to initiate the form.
• Approval Process: The form will already have a list of required signatures built into it. The form will move from person to person in the order of signatures displayed in the approval process table. If this order does not look right, edit your form to update the order of signatures. 
  • You will notice a By Email option for each signature. This should only be selected if the form is to be signed by someone who does not have an Experiential Learning Cloud account. Checking this box will allow students to enter any email address when delegating the form. If this box is not checked, students will only be able to route it to the site supervisor(s) and/or faculty members who have supervision over them in Experiential Learning Cloud. 
• To add an additional reviewer to approve the assignment after all signatures have been applied, click the (+) at the top of the Approval Process table, then select the specific person or reviewer group that will review the assignment. 

Optional Additions 

• Instructions: Click the ellipses to add any additional instructions that students may need to complete the assignment.
• Linked Assignments: Assignments can be linked to each other. This is helpful if you have multiple connected assignments.
• Prerequisites: If this assignment has prerequisites (assignments that must be completed prior to the student beginning this assessment), select them here.
  • NOTE: The prerequisite assignment must be entered in the assignment builder first before it will appear in this list.
• Start Date: This field will default to the Start Date of the class. If the student cannot start the assignment until a specific date, enter that date here. 
• Due Date: This field will default to the End Date for the class or program. If the due date is different from the end date, enter the due date here.
• Assigned to: This field determines which students will see the assignment in their assignments workspace. Assignments can be assigned to ...
  • All class or program members.
  • No class or program members. 
  • Specific class or program members -- If there is a unique assignment that is assigned to only one or some of the students in the class, select this option.                         
• Required or Optional: Assignments can be ...
  • Required for all members of the class or program (this is the default setting).
  • Optional for all members of the class or program.
  • Required for specific members of the class or program.
• Assignment attachments (visible to all assignees): If you'd like to provide an attachment with the assignment that all assignees can access, such as a work sample or additional instructions, you can upload that attachment here.

Once all assignments have been added to your class template, you can copy them to a class.
