Problems Opening a Form

Solution

Solution 1: Select a Program

1. Locate the red Program: box on the right side of the page.
2. Select the appropriate program from the dropdown menu.
3. Reload or reopen the form if needed.

Once the correct program is selected, the form should load normally.

Troubleshooting

Subject Not Selected

If the form still does not load after selecting a program, a subject may also need to be selected. The subject identifies the person the form applies to, such as:

• The current user
• A supervisor
• A supervisee

Resolution:

1. Locate the subject selection area on the left side of the page.
2. Select the appropriate subject.
3. Reload or reopen the form if needed.

After both the program and subject are selected, the form should load correctly.

Missing Start or End Date

Some forms, such as contracts or agreements, require both a Start Date and an End Date before the form can load. Once the end date passes, the form is automatically marked inactive and the information is no longer considered valid.

A required end date is indicated when the date fields appear as mandatory within the form.

Resolution:

4. Select the Start Date.
5. Select the End Date (labeled Stop).
6. Click OK.

After both dates are entered, the form should load correctly.

Best Practices

Important: Select the Subject’s Program

When completing a form for another person (such as a supervisee), always select the program associated with that subject, not the program associated with the current user.

Selecting the wrong program can prevent the subject from accessing the form and may cause the form to remain stuck in the workflow.

Example: A site supervisor is completing an evaluation for a supervisee enrolled in the School Counseling program. Even if the supervisor is personally enrolled in both School Counseling and Clinical Mental Health, the form must be associated with School Counseling because that is the supervisee’s program.
