About this form:
This form is intended to be completed by an authorized site representative from a field agency. It is designed to provide any information which would be helpful to students as they apply to agencies for their field education experiences.

Many of the fields on this form are “not required” so agency representatives can skip over the information that is not relevant to their agency, but the more they can provide, the more helpful this form will be to your students.

These forms can also be completed by program administrators and field placement faculty, if your program prefers to provide this information rather than asking the agencies.

Experiential Learning Cloud Best Practice:
Create custom filters so that students and field faculty can search for agencies by criteria on this form such as Type of Site, Client Populations Served, Schedule, etc. Talk to an Implementation Specialist about utilizing this feature in your Experiential Learning Cloud database.
