If this form is included in a placement update process, students will be able to initiate a placement withdrawal by completing this form. Once the form is completed and signed by the student and the placement coordinator, the placement coordinator will be able to approve the student's placement withdrawal. If the withdrawal is approved, the student's placement at the site will end on the updated site end date, as listed on the form.
