• Hours Reports for the first half of the semester
  • Students can report on hours on a weekly or monthly basis, or they can simply report their hours once at midterm and once at the end of the semester

• Midterm Evaluation
  • The tasks students committed to in their learning plan will be copied into our standard midterm evaluation so that field instructors can easily assess student progress

• Hours reports for the latter half of the semester

• End of Semester Hours Summary
  • We encourage the use of an End of Semester Hours Summary so that you and your students have one definitive document with all of the hours they accrued throughout the semester, signed by a site supervisor

• Final Evaluation
  • The tasks students committed to in their learning plan and the midterm ratings provided by the field instructor will be copied into our standard final evaluation form so that field instructors can easily assess student progress
