Overview

The following types of forms are good candidates for Form Builder Lite:

• Basic forms that are completed and signed by a single user
• Forms that do not need extra logic (viewing permissions, hidden fields, etc)
• Forms that do not need rubric reporting

To build our full-featured “pro” forms, you will need the Program Setup role. Learn more here.

How to Access

To access this feature, you need to have either the Form Builder Lite or Program Setup role.

Once you have either role, you will then have access to Form builder Lite under your Program Setup| Customization in the Custom Forms tab.

Creating a Custom Lite Form

1. To start a new form, go to your Program Setup |Customization Environment and select the Forms tab on the top left.
2. Click on the Add (+) button in the top right corner of the table to begin creating a new form.
3. Choose “Create Lite Form” to create a new, basic form.

4. Click on the plus icon at the top right corner of the table. Form Builder Lite will then open. You will need to name the form and select a document type in order for the form to load and save.

5. The document type will typically be “Other” under Staff/Student. If you select a Site document type, students and faculty will not be able to edit the form, only program administrators and site representatives (ASRs) will.
6. The document type you select will determine whether a document has an end date or not. If your chosen document type has an end date, the person who initiates the form will be prompted to enter an end date before they can fill out the form. This is helpful in the case of agreements or liability insurance - so that the end date can be tracked in the system. The following document types have end dates:
   1. Activity
   2. Liability Insurance
   3. License
   4. Partnership Agreement (All versions)
   5. Site Liability Insurance (All Versions)
   6. Student Agreement/Goals
   7. Treatment Plan
7. Documents can also be private or public - this is most relevant in the case of the "Site Versions" of documents. If a document is a "site version" document, it means that it can be stored in the site's record. Public site documents can be seen by students, while private site documents are hidden from students and can only be seen by program faculty and site staff.
8. The form will open with a text box prompting you to input instructions.

Best Practice:

If students will be completing this form, you should put instructions on how they are to complete the form, especially if you plan on using the form within an assignment.

Form Builder Lite Features

1. Once the form opens, you can add multiple field types using the tool menu on the right side.

2. To add a new type of input field, click on the plus icon. This will give you all field options that you can add to your form.

• The Short Answer field allows users to input text answers that are 150 characters or less. You can put your question or instructions for the short answer field that reads "Untitled."
• The Paragraph field is similar to the Short Answer, but answers can be longer than 150 characters. In any field, you can click on the Settings icon to add a description of the field, as well as inactivate the field on the form.
• The Multiple Choice field allows you to add radio buttons that you can title yourself. You can also add an "Other" option, in which the user can input a different option than the ones provided on the form.
• Checkboxes are another field option for your custom form, similar to the multiple choice option. Checkboxes and multiple choice fields allow for multi-selection if you select that feature under the setting icon for the field. This allows the user filling out the form to select more than one option.
• In the Dropdown field, you can add as many options or selections as you would like, which will then appear in a dropdown menu for the user completing the form.
• The Date field is typically used to indicate the start or end dates of a placement of course. You do not need to add a date field for signatures, as the date of the signature is automatically shown.
• The Signature field allows you to add one or more signatures to the form.
