Checkbox fields are very similar to Multiple Choice fields in that all of your options are visible on the form, the difference being you can select more than one option on a checkbox field. 

How to add a Checkbox Field:

1. Click the plus icon in your tool menu.

2. Select Checkboxes

3. Enter your field question or title.

4. Click Add option

   • Enter your option name

   • Keep clicking Add option until all your options are listed

   • Click Add other if you want an Other option with a text entry field 

5. You can change the order of the listed options by clicking and dragging them within the list
