If you used a form placeholder, but your communication recipient did not receive the link to the form, check these steps to troubleshoot:

• Ensure the form design includes an action (submit) button with read/write or editable permission for the role of the communication recipient.
• Check that the action button has not yet been actioned/clicked.
• Ensure that the form was assigned correctly.
• Confirm the recipient is actually in Sonia as the role of the person who is supposed to complete the form (e.g., has been assigned as Mentor on the Worksheet).

NOTE: The form placeholders include forms that have not yet been actioned. So, if the recipient has completed the form, the form will not be included in the placeholder.
